PLEASE READ THE FOLLOWING CONDITIONS CAREFULLY
You certify that you are a member in good standing of the Bar of the State of New Jersey and a member of the Union County Bar Association.
You will agree to pay a $200.00 registration fee for calendar year 2023, which entitles you to choose (4) specialty areas from the categories listed (subcategories do not count) on the reverse side of this application. You may select additional panels. Each additional panel will cost $50.00. For example, if you sign up for six panels your LRS registration fee will be $300.00. All checks should be made payable to the Union County Bar Association.
You will agree to charge $35.00 for the first half hour’s consultation with clients referred by the Union County Lawyer Referral Service, with the understanding that you will advise the client of what the charges will be if the consultation runs over the half hour.
You agree to submit a copy of the declaration page of your malpractice insurance policy showing coverage of at least $100,000/300,000 and naming the Union County Lawyer Referral Service as a holder of the certificate.
You agree to execute a certification on the number of fee arbitration disputes or ethics complaints currently pending and you further agree to promptly notify the UCBA Lawyer Referral Service if any such grievances are filed while you are serving on this panel.
